Welcome to Lumera Recovery Pathways. It is our privilege to get to know you and share this journey ahead with you. Navigating through the Alcohol and Drug treatment services can often be daunting, tiring and frustrating. At Lumera, we hope to ease this burden by delving into our many years of experience to find the best path forward.
Lumera Recovery Pathways works with our clients to achieve their desired outcomes by skilfully assessing their unique situations, offering personalised education, making recommendations, liaising with their current treatment team, facilitating referral to other services, and most importantly tailoring a recovery plan individualised accurately to our client’s needs. Furthermore, we offer ongoing recovery coaching support to assist clients in maintaining change. Challenges are guaranteed to come, so we want to be there to walk this path with our clients when they arrive.
Consent to Service
For Lumera Recovery Pathways to begin working with you, we require you to consent to the service. By signing this form you will consent to engaging Lumera Recovery Pathways.
Privacy and Confidentiality
Lumera Recovery Pathways is required to collect sensitive client information to provide a service to its clients. Lumera Recovery Pathways deeply values our clients’ information and will commit to protecting their privacy. Lumera Recovery Pathways is a confidential service. Any information you share with us will be kept private between you and our service unless you give us authorisation to do so. However, if we were of the belief that you were at harm to yourself or others, for the purposes of preventing harm, we would have a duty of care to release some of your information to emergency services and/ or loved ones. This would also extend onto the welfare of any children affected.
By signing this form, you acknowledge and agree that Lumera Recovery Pathways can obtain, store, share and disclose information that is relevant to your treatment journey in accordance with the Lumera Recovery
Pathway privacy policy.
Service Fees
Lumera Recovery Pathways has a fee-for-service. Our payment scheduling is available on our website at Lumerapathways.au. The initial assessment/ consultation requires an upfront payment via our website and booking system. Supplementary appointments can be paid for via Eftpos Air at the appointment and are required to be made on the day. Please note, cash is not accepted. There are no Medicare, Private Health insurance and NDIS rebates currently available.
Cancellations
Appointments cancelled at a minimum of 24 hours notice prior to the appointment will be fully refundable or transferrable for upfront payments. Cancellations with less than 24 hours notice will result in a 50% cancellation fee for any upfront or pending payments.
Staying in Touch
Lumera Recovery Pathways believe that our clients are centre in their recovery plan. To progress these plans, Lumera wants to keep our clients informed and up to date along the way. We pride ourselves in our communications with our clients and other service providers. We use face to face appointments, telephone calls, video conferencing, text messaging, and emails to provide excellence in our communication
Authority to Release and Obtain Information
To optimise my pathway forward with Lumera Recovery Pathways, I consent to Lumera Recovery Pathway’s liaising with the following people and services to provide part or all of my information and/or obtain information relevant to my treatment pathways and goals.
