Lumera Recovery Pathways provides specialist outpatient alcohol and other drug support for individuals and families across the continuum of recovery.
Our Clinical Nurse Specialist model complements existing healthcare providers through comprehensive assessment, individualised recovery planning, collaborative care coordination and continuity of support between appointments. We work alongside GPs and other referrers to strengthen engagement, improve communication and support sustainable recovery outcomes.
* Note this is a fee-for-service provider. No rebates are currently available. Lumera Recovery Pathways does not prescribe medication during any stage of the journey.
The Client Journey.
Every client follows a structured recovery pathway tailored to their individual needs. From the initial consultation through to long-term recovery, Lumera provides comprehensive assessment, personalised recovery planning, collaborative care coordination and continuity of support between appointments. Care is adjusted over time and delivered in partnership with GPs and other treating clinicians to support sustainable recovery outcomes.
How Feedback Improves Care.
Continuous quality improvement is embedded within the Lumera model of care. We invite clients to share feedback following each session and at the conclusion of treatment, allowing us to evaluate outcomes, refine our service and strengthen the care we provide. Their feedback consistently reinforces the importance of compassionate conversations, individualised recovery planning and continuity of support throughout recovery.
